WHEN: Hours are 9 a.m.-3 p.m., Tuesday, Dec.1, through Thursday, Dec. 3.
WHERE: Karuk Tribal Wellness Center, 537 Jacobs Way, Happy Camp, CA 96039.
The deadline to register with FEMA for disaster assistance under DR-4569 is Dec. 16.

- Download the FEMA Mobile App for smartphones.
- Call 800-621-3362(TTY: 800-462-7585) between 7 a.m. to 8 p.m. PST, seven days a week. Multilingual operators are available.
